Warranty Request on G34

I have an Arris G34 that I need to submit for warranty request.  How do I do this?  We have already gone through all trouble shooting steps with our provider as well as online through Arris with no luck.  We only have an orange amber light with no signal.

Our device is under warranty until Dec 2025.  Please assist.
