WiFi Network Names and Password of 2.4/5GHz are different ?

I just got a new modem and for some reason, the 5g network name is different from the 2.4. The password that I am using find for the 2.4 is not working for the 5g network.  Any ideas?

Could you help us with the model number of the device? 

So it will be easy to assist further on the WiFi Network (2.4GHz /5GHz)
