As the Title says, checking my event log after provisioning was complete showed SW Upgrade Failed. Is this an issue with Spectrum not pushing the correct firmware?

and the internet is working? firmware updates are coming directly from your internet provider so yes it’s look like something happened in their system when they were pushing the boot file
