My ex and I split up, his name was on the internet account so I had to deactivate and create my own account. I am still using the same Router/modem as before, but it will not connect to the new internet service. Do I have to deactivate something on the modem or do I need to transfer the equipment into my name also? If so, how do I go about doing this?

To change the service completely from one contact to another you need to reach out to your internet service provider to activate account under your name. If your looking details to change the wifi credentials then you need to login to your modem GUI or web interface or you can perform hard reset on the modem by pressing the reset button on the modem for 15-3secs so after resetting the modem will go to default setup any custom settings will be removed so use the default network name and password to connect to your network.
