Periodically the modem won’t display the caller ID on the phone, just says incoming call. Ate the same time the voice on the phone call becomes scratchy. Reboot modem and all is well for a while again.

T25 is just a voice cable modem so it doesn't have any advanced settings on the modem setting page or GUI. For this issue we can help you with the basic trouble shootings steps like, you can try restarting your phone using the setting option and setup again, also reset modem before restart phone this will help your modem to get configured and issue will be fixed. For the caller ID issue you need to check with phone manufacturer.
