New Surfboard S34 Modem will not activate w ISP

I tried to self-activate the modem with my ISP. It failed multiple times. I tried to activate the modem with their tech support as failed as well. The modem is on their approved list. Their support looked up the MAC address and no record associated with that MAC address. I also was able to confirm that the MAC address has no record and it is NOT associated with any known manufacturer.

I bought the modem from Amazon and it was sold directly by Amazon.

Could you please provide the name of your internet service provider (ISP) and let us know the current status of the LED lights on your modem?

If you have any other coaxial outlets available, try plugging the modem into one of those. Make sure that the modem displays solid blue lights, which indicate an active connection. Once you have a solid connection, you can proceed with the self-activation process.
